Check-in Forms

For your convenience, the following documents can be printed and completed prior to your first visit to Chesapeake Pet Resort. Questions about these documents can be answered by contacting Chesapeake Pet Resort at wecare@chessiepets.com or 301-373-3400.

On Check-In day, please bring your pet in on lead, or in a pet carrier, and a copy of your current vaccination records, if not already on file with us. New customers are asked to print the forms below and bring the completed copies in on Check-In day. Please arrive at least 15 minutes prior to any closing times to allow time for pet check-in. All food, treats, and medications should be labeled with your last name, however, we do not guarantee return of any items (except unused medication), due to the huge volume of materials that typically arrive on any given day. Please do not bring bedding, laundry, or bowls from home. We have all of those supplies, plus plenty of toys needed for your pet’s comfort.
